ABSTRACT

Abstract Background: Arboviral diseases are a group of infectious diseases caused by viruses transmitted by arthropods, mainly mosquitoes. These diseases, such as those caused by the dengue (DENV), Zika (ZIKV), chikungunya (CHIKV), and yellow fever (YFV) viruses, have a significant impact worldwide. In this context, entomological surveillance plays a crucial role in the control and prevention of arboviruses by providing essential information on the presence, distribution, and activity of vector mosquitoes. Based on entomological surveillance, transovarian transmission provides information regarding the maintenance and dissemination of arboviruses. The objective of this study was to detect these arboviruses in Goiânia, Goiás, and analyze the occurrence of transovarian transmission. Methods: Aedes aegypti eggs were collected from different regions of Goiânia and cultivated under controlled laboratory conditions until the emergence of adult mosquitoes. Adult females were grouped into pools containing their heads and thoraxes. These pools were subsequently evaluated using reverse-transcription quantitative polymerase chain reaction (RT-qPCR) assay. Results: A total of 157 pools (N=1570) were analyzed, with two pools testing positive for CHIKV and one pool testing positive for ZIKV, indicating that the offspring resulting from transovarian transmission are potentially infectious. Conclusions: In summary, the demonstration of the vertical transmission mechanisms of CHIKV and ZIKV in A. aegypti serves as an alert to health authorities, as these diseases are still underreported, and their primary urban vector has likely acquired this capacity, contributing to the dissemination of these infections.

ABSTRACT

Resumo Num cenário epidêmico ainda preocupante, a prevenção da Transmissão Vertical (TV) do HIV impõe problemas complexos, devido as vulnerabilidades individual, social e moral das mulheres vivendo com o vírus, somadas às fragilidades da rede de saúde. A partir de um caso emblemático, este estudo buscou compreender os desafios bioéticos do cuidado para a prevenção da TV do HIV no âmbito do Comitê de Porto Alegre/RS. Os eixos analíticos desenvolvidos refletem sobre como a produção do cuidado se articula, por um lado, com discursos e práticas relacionais pautadas no gênero e interseccionadas por raça e classe social e, por outro, com vulnerabilidades programáticas das políticas de saúde. Vislumbrou-se um processo de extrema estigmatização, em que as poucas ofertas para as mulheres cisgênero se dirigiam à regulação reprodutiva e perpetuavam dinâmicas de violência estrutural. Discute-se caminhos para a construção de um cuidado que incorpore a perspectiva decolonial e busque produzir equidade e justiça social ao reconhecer as trajetórias das mulheres.


Abstract In a still worrying epidemic scenario, the prevention of Vertical Transmission (VT) of HIV poses complex problems, due to the individual, social, and moral vulnerabilities of women living with the virus, in addition to the weaknesses of the health network. Based on an emblematic case, this study sought to understand the bioethical challenges of HIV VT prevention in the scope of the Porto Alegre/RS Committee. The analytical categories developed reflect on how the production of care is articulated, on the one hand, with relational discourses and practices based on gender and intersected by race and social class and, on the other, with programmatic vulnerabilities of health policies. A process of extreme stigmatization was revealed, in which the few offers for cisgender women were aimed toward reproductive regulation and perpetuated dynamics of structural violence. We discuss ways of building care that incorporates a decolonial perspective and seeks to produce equity and social justice by recognizing women's trajectories.

ABSTRACT A 7-week-old male delivered by cesarean section presented with a positive serology for dengue along with preretinal and retinal hemorrhages, vitreous opacities and cotton wool spots. The patient and his mother had positive serologies for Non Structural Protein 1 (NS1) by ELISA. Retinal and vitreous findings improved over a sixteen-week period. Spectral domain optical coherence tomography (OCT) showed preserved macular architecture. In this case report, we suggest that retinal and vitreous changes may be the ocular presenting features of vertically transmitted dengue in newborns, and that those findings may resolve with no major structural sequelae.


RESUMO Neonato de 7 semanas, do sexo masculino, nascido de parto cesárea, apresentou sorologia positiva para dengue com hemorragias retinianas e pré-retinianas, opacidades vítreas e manchas algodonosas. O paciente e sua mãe haviam apresentado sorologias positivas para Non Structural Protein 1 através de ELISA. Achados na retina e no vítreo melhoraram em um período de dezesseis semanas. O exame de tomografia de coerência óptica de domínio espectral demonstrou arquitetura macular preservada. Neste relato de caso, sugerimos que alterações na retina e no vítreo podem ser os achados oculares aparentes em neonatos com infecção vertical por dengue, e que estes podem se resolver sem maiores sequelas estruturais.

Abstract Caryocar brasiliense Camb. (Malpighiales: Caryocaraceae) trees are widely distributed throughout the Cerrado ecosystem. The fruits of C. brasiliense trees are used by humans for food and as the main income source in many communities. C. brasiliense conservation is seriously threatened due to habitat loss caused by the land-use change. Sucking insects constitute an important ecological driver that potentially impact C. brasiliense survival in degraded environments. In addition, insects sampling methodologies for application in studies related to the conservation of C. brasiliense are poorly developed. In this study, sucking insects (Hemiptera) and their predators were recorded in three vertical strata of Caryocar brasiliense canopies. The distribution of sucking species showed vertical stratification along the canopy structure of C. brasiliense. The basal part of the canopy had the highest numbers of sucking insects Aphis gossypii (Glover 1877) (Hemiptera: Aphididae) and Bemisia tabaci (Genn. 1889) (Hemiptera: Aleyrodidae), and their predators Chrysoperla sp. (Neuroptera: Chrysopidae), spiders (Araneae), and Zelus armillatus (Lep. & Servi., 1825) (Hemiptera: Reduviidae). Predators' distribution follows the resource availability and preferred C. brasiliense tree parts with a higher abundance of prey.

Insetos sugadores constituem um importante fator ecológico que, potencialmente, afeta o fitness de C. brasiliense em ambientes degradados. Além disso, as metodologias de amostragem de insetos para aplicação em estudos relacionados à conservação de C. brasiliense são pouco desenvolvidas. Neste estudo, o número de insetos sugadores (Hemiptera) e seus predadores foram avaliados em três estratos verticais do dossel de C. brasiliense. A distribuição das espécies sugadoras apresentou estratificação vertical ao longo da estrutura do dossel. O estrato basal do dossel apresentou o maior número de insetos sugadores Aphis gossypii (Glover 1877) (Hemiptera: Aphididae) e Bemisia tabaci (Genn. 1889) (Hemiptera: Aleyrodidae), e seus predadores Chrysoperla sp. @#The functional health and stability of the oral and maxillofacial system is one of the basic goals of orthodontic treatment. Currently, it is believed that, in general, the condyle is located in the center of the joint fossa when the mandible is in an intercuspal position (ICP) in healthy normal people. At this time, the function of the temporomandibular joint (TMJ) is stable. Due to orthodontic tooth movement and subsequent occlusal changes, patients with malocclusion may experience related remodeling of the temporomandibular joint, especially changes in the position of the condyle. The position of the mandibular condyle is traditionally evaluated using a condylar position indicator. However, this method lacks consistency in obtaining condylar position changes. In recent years, in the clinical application of orthodontic treatment, cone beam computed tomography (CBCT) has become the first choice for examination. CBCT can accurately measure the interarticular space and determine changes in condylar position. This article reviews the CBCT assessment of condylar position and related research on condylar position changes in patients with malocclusion before and after orthodontic treatment. The literature review results indicate that there are differences in the condylar position of patients with different malocclusions, and the condylar position may also change before and after orthodontic treatment. With a lower radiation dose, CBCT has higher accuracy in evaluating the condylar position in patients with malocclusion who undergo orthodontic treatment, thus promoting further study of the mechanism of condylar position changes in patients with malocclusion in the future and providing more accurate and personalized guidance for patient treatment.

Introducción: El estado de ánimo y la autoeficacia presentan estrecha relación con el rendimiento deportivo. En atletismo, contar con un control exhaustivo del entrenamiento físico y mental supone asentar las bases del máximo rendimiento, siendo el salto vertical un excelente elemento de control. Sin embargo, su relación con los aspectos psicológicos durante los entrenamientos se ha divulgado poco en la literatura. Este artículo tiene como objetivo analizar la relación entre el perfil de estado de ánimo y la autoeficacia durante saltos verticales en deportistas juveniles de la preselección nacional cubana de atletismo. Método: Se evaluó el estado de ánimo y la autoeficacia antes de ejecutar un salto con contra movimiento (CMJ), presentándose el valor de 40 cm como elemento de máximo rendimiento en atletas de élite. Resultados: Se encontró correlación significativa entre el factor ansiedad del POMS y variables del CMJ. Además, se encontró correlación negativa entre la autoeficacia y la ansiedad, aunque no significativa (p > .05). Conclusiones: Los deportistas con un estado de ánimo de puntuación media de ansiedad, aunque no sobreelevada (puntaje 2 sobre 4) antes de la ejecución de saltos verticales, obtuvieron mejores resultados de CMJ, consiguiendo un mayor rendimiento tanto en la altura de salto, como en la velocidad, fuerza y potencia del salto. Sin embargo, no se hallaron correlaciones significativas entre la autoeficacia y el estado de ánimo, ni con variables de salto.


Introduction: Both mood and self-efficacy seem to be components closely related to the athlete's performance. In athletics, to have an exhaustive control of training from the physical and mental point of view is to lay the foundations for sporting success. Exercises such as the vertical jump are an excellent element of control of athletic performance. However, their relationship with psychological aspects during training has been little reported in the literature. Our objective was to analyze the relationship between the mood profile and self-efficacy during vertical jumps in youth athletes of the Cuban national preselection of athletics. Method: Mood and self-efficacy were evaluated before executing a countermovement jump (CMJ), presenting the 40-cm value as the element of maximum success in elite athletes. Results: A significant correlation was found between anxiogenic mood and vertical jump variables. In addition, a negative correlation was found between self-efficacy and anxiety, although not significant (p > .05). Conclusions: Athletes with a medium anxiety score mood although not overelevated (score 2 out of 4) before the execution of vertical jumps obtained better CMJ results, achieving higher performance in both jump height, speed, strength and power of the jump. However, no significant correlations were found between self-efficacy and mood, nor with the jumping variables.

Introducción. La cirugía bariátrica es una estrategia válida de tratamiento en obesidad severa. El objetivo de este estudio fue evaluar la reducción de peso y la resolución de comorbilidades comparando dos técnicas quirúrgicas, baipás gástrico en Y de Roux y manga gástrica. Métodos. Estudio descriptivo de tipo analítico que incluyó pacientes con obesidad grados II y III. Se analizaron variables demográficas y perioperatorias, y las comorbilidades asociadas a la obesidad. La reducción del peso se evaluó con el porcentaje de pérdida del exceso de peso. Se realizó un análisis descriptivo univariado, usando medianas, rangos intercuartílicos, frecuencias y proporciones. Se usaron las pruebas de U de Mann-Whitney y Chi cuadrado para el análisis de grupos. Un valor de p<0,05 fue considerado estadísticamente significativo. Resultados. Fueron incluidos 201 pacientes. La mediana del porcentaje de pérdida del exceso de peso a 18 meses fue de 77,4 % para el grupo de baipás gástrico en Y de Roux vs 69,5 % para el grupo de manga gástrica (p=0,14). La mayoría de los pacientes presentaron resolución o mejoría de la hipertensión arterial (76 %), diabetes mellitus (80 %), dislipidemia (73 %), apnea del sueño (79 %) y artropatías (94 %), sin diferencia significativa según la técnica quirúrgica empleada. La tasa de complicaciones mayores fue del 1,9 %. No se presentó mortalidad. La mediana de seguimiento fue 28 meses. Conclusión. El baipás gástrico en Y de Roux y la manga gástrica son procedimientos muy seguros y efectivos para la reducción del exceso de peso y la resolución de las comorbilidades asociadas a la obesidad


Introduction. Bariatric surgery is a valid strategy of treatment for severe obesity. The aim of this study is to evaluate weight loss and resolution of comorbidities comparing two procedures, Roux-en-Y gastric bypass and sleeve gastrectomy. Methods. Descriptive study of analytical type that included patients with obesity grades II and III. Demographic and perioperative variables were analyzed. The weight reduction was evaluated among others with the percentage of excess of body weight loss. Comorbidities associated with obesity were also analyzed. A univariate descriptive analysis was performed, using medians, interquartile ranges, frequencies, and proportions. The Mann-Whitney U and Chi squared tests were used for analysis of groups. A value of p <0.05 was considered statistically significant. Median follow-up was 28 months. Results. A total of 201 patients were included in the analysis. The median percentage of excess of body weight loss at 18 months was 77.4% for Roux-en-Y gastric bypass group vs 69.5% for sleeve gastrectomy group (p=0.14). The majority of patients presented resolution or improvement of hypertension (76%), diabetes mellitus (80%), dyslipidemia (73%), sleep apnea (79%), and arthropathy (94%), without significant differences according to the surgical technique used. Major complication rate was 1.9%. There was not mortality. The median follow-up was 28 months. Conclusion. Roux-en-Y gastric bypass and sleeve gastrectomy are both very safe and effective procedures for excess weight reduction and resolution of comorbidities associated with obesity

ntroducción: Las infecciones perinatales pueden transmi-tirse al feto y al recién nacido. Sífilis, VIH y hepatitis B deben tamizarse durante la gestación.Objetivo: Conocer la incidencia, prevalencia y manejo de sífilis, VIH y hepatitis B en el binomio madre/hijo. Comparar resultados con estadísticas oficiales.Materiales y métodos: Estudio retrospectivo, descriptivo y analítico, mediante revisión de historias clínicas del Sana-torio de la Cañada y Hospital Pasteur, Villa María, Córdoba. Período 01/12/2020 al 31/07/2021. Resultados: Se estudiaron 870 embarazos, la incidencia de sífilis materna fue 52,87/1000 embarazos, 76,1% de las gestantes eran menores de 30 años y hubo 41% de diag-nósticos tardíos. La incidencia de sífilis congénita fue de 18,3/1000 RN vivos. La incidencia de VIH materno fue de 6,89/1000 embarazos, 66,7% tenían menos de 30 años y el 77,7% tuvo carga viral indetectable al parto. El 100% de los RN expuestos fueron estudiados, todos con carga viral indetectable al nacimien-to. No hubo casos de hepatitis B.Conclusión: 6,3% de las embarazadas presentaron al me-nos una serología reactiva y el mayor porcentaje diagnós-tico se centró en menores de 30 años. La incidencia de sífilis congénita superó la provincial y nacional (18,3 vs. 1,18 vs. 1,14). El porcentaje de positividad de VIH materno superó al provincial. No hubo transmisión vertical de VIH al nacimiento. La prevalencia de hepatitis B fue menor a las oficiales


Introduction: Perinatal infections can be transmitted to the fetus and new-born. Syphilis, HIV and Hepatitis B must be monitored during pregnancy.Objective: To know incidence, prevalence and management of syphilis, HIV and Hepatitis B in the mother/child binomial. To compare results with official statistics.Materials and methods: Retrospective, descriptive and analytical study, through the review of medical records from Sanatorio La Cañada and Hospital Pasteur in Villa Maria, Cordoba. Period 12/01/2020 to 07/31/2021.Results 870 pregnancies were studied, the incidence of maternal syphilis was 57.87/1000 pregnancies, 76.1% of pregnant women were under 30 years old, and there were 41% late diagnoses. The incidence of congenital syphilis was 18.3/1000 live newborns. The incidence of maternal HIV was 6.89/1000 pregnancies, 66.7% were women under 30 years old and 77.7% had undetectable viral load at birth. 100% of the exposed newborns were studied, all with undetectable viral load at birth. There were no cases of Hepatitis B.Conclusion: 6.3% of pregnant women presented at least one reactive serology and the highest diagnostic percentage was focused on those under 30 years old. The incidence of congenital syphilis exceeded the provincial and national data (18.3 vs 1.18 vs 1.14). The percentage of maternal HIV positivity was superior to the provincial one. There was no vertical transmission of HIV at birth. The prevalence of Hepatitis B was less than the official ones.

SUMMARY: The aim of the article is to determine the differences in morphological characteristics and jumping abilities between female volleyball players who play for different national teams. The sample consists of 48 elite female volleyball players from four different national teams which participated in European championship qualifications. The variables studied were: body height, mass, body mass index, standing reach and spike reach. The results show differences in body mass index, standing reach and spike reach. These differences are related to the needs of the different positions with regard to the actions they execute. In conclusion morphological parameters are important components of performance in many sports (volleyball). Different sports disciplines require different body parameters and body structure for maximum performance.


El objetivo del artículo fue determinar las diferencias en las características morfológicas y habilidades de salto entre las jugadoras de voleibol que juegan en diferentes selecciones nacionales. La muestra consta de 48 jugadoras de voleibol de élite de cuatro equipos nacionales diferentes que participaron en las clasificaciones del campeonato europeo. Las variables estudiadas fueron: altura corporal, masa, índice de masa corporal, alcance de pie y alcance de remate. Los resultados muestran diferencias en el índice de masa corporal, el alcance de pie y el alcance de punta. Estas diferencias están relacionadas con las necesidades de los distintos cargos en cuanto a las acciones que ejecutan. En conclusión, los parámetros morfológicos son componentes importantes del rendimiento en muchos deportes (voleibol). Las diferentes disciplinas deportivas requieren diferentes parámetros corporales y estructuras corporales para un rendimiento máximo.

ABSTRACT

As próteses parciais removíveis (PPRs) são uma alternativa de tratamento viável na prática clínica para reabilitar arcos parcialmente desdentados. Entretanto, o planejamento dessas próteses e preparo bucal prévio são frequentemente negligenciados. O presente artigo tem como objetivo relatar o caso clínico de uma reabilitação oral com PPRs superior e inferior após abordagem multidisciplinar, de modo a enfatizar as fases de um planejamento criterioso e de preparo prévio dos dentes pilares, visando o sucesso e a longevidade da reabilitação. Paciente do sexo masculino de 57 anos de idade compareceu à clínica da Faculdade de Odontologia de Bauru, Universidade de São Paulo, queixando-se da estética de seu sorriso e de algumas ausências dentárias. O indivíduo utilizava uma PPR provisória inferior insatisfatória e apresentava perda de dimensão vertical de oclusão (DVO). Após o exame clínico, radiográfico e estudo do caso em articulador semi-ajustável, realizou-se o planejamento com abordagens restauradoras, endodônticas, periodontais e protéticas. Após tratamento periodontal, foi realizada endodontia dos elementos 12, 15 e 47, confecção de núcleos e coroas nos dentes 12 e 15, restauração a nível gengival para apoio residual no dente 47, restaurações nos dentes 11, 13, 22, 24 e 44 e, por fim, a confecção das PPRs superior e inferior. A abordagem multidisciplinar utilizada neste caso clínico viabilizou o restabelecimento da DVO e possibilitou o sucesso da reabilitação protética(AU)


Removable partial dentures (RPDs) are a viable treatment alternative in clinical practice to rehabilitate partially edentulous arches. However, the planning of these dentures and prior oral preparation are often neglected. This article aims to report the clinical case of an oral rehabilitation with upper and lower RPDs after a multidisciplinary approach, to emphasize the phases of careful planning and prior preparation of the abutment teeth, aiming at the success and longevity of the rehabilitation. A 57-year-old male patient came to the clinic of the Bauru School of Dentistry, University of São Paulo, complaining about the esthetics of his smile and some missing teeth. The subject was using an unsatisfactory lower provisional prosthesis and had a loss of vertical dimension of occlusion (OVD). After the clinical and radiographic examination and the case study in a semi-adjustable articulator, planning was carried out with restorative, endodontic, periodontal and prosthetic approaches. After periodontal treatment, endodontics were performed on elements 12, 15 and 47, creation of cores and crowns on teeth 12 and 15, restoration at the gingival level for residual support on tooth 47, restorations on teeth 11, 13, 22, 24 and 44 and, finally, the making of the upper and lower PPRs. The multidisciplinary approach used in this clinical case enabled the restoration of the OVD and enabled the success of the oral rehabilitation(AU)

ABSTRACT

O envelhecimento bucal precoce apresenta cada vez uma maior incidência nos consultórios odontológicos e sua degradação aos tecidos orais exige uma atenção por parte do cirurgiãodentista devido ao seu alto grau de complexidade. Esse envelhecimento precoce é causado principalmente por hábitos parafuncionais, dieta ou ambos. Suas consequências são desgastes patológicos dos tecidos dentários, extrusão passiva, perda de dimensão vertical e comprometimento estético e funcional. Por conta disso, o presente estudo teve como objetivo realizar um relato de caso clínico em um paciente jovem e com queixa estética como consequência de hábitos parafuncionais, aonde foram realizados uma reabilitação oral envolvendo aumento da dimensão vertical de oclusão através de Table Tops sem desgastes dentários e restaurações estéticas, ambas com resina composta. Essa reabilitação devolveu a DVO da paciente, trazendo conforto, contatos estáveis, guias de desoclusão e satisfação estética e funcional por parte da paciente(AU)


Early oral aging has an increasing incidence in dental offices and its degradation to oral tissues requires attention from the dentist due to its high degree of complexity. This premature aging is mainly caused by parafunctional habits, diet, or both. Its consequences are pathological wear of dental tissues, passive extrusion, loss of vertical dimension and aesthetic and functional impairment. Because of this, the present study aims to carry out a clinical case report in a young patient with an aesthetic complaint as a result of parafunctional habits, where an oral rehabilitation was carried out involving an increase in the vertical dimension of occlusion through Table Tops without dental wear and aesthetic restorations, both with composite resin. This rehabilitation returned the patient's OVD, bringing comfort, stable contacts, disocclusion guides and aesthetic and functional satisfaction on the part of the patient(AU)

ABSTRACT

A sífilis persiste como um problema de saúde pública, sobretudo pelos entraves existentes no enfrentamento da sífilis gestacional e congênita. Considerando que a ocorrência dessas infecções se relaciona a fatores maternos e programáticos, este estudo buscou analisar as características epidemiológicas do binômio mãe-filho exposto à sífilis e sua distribuição espacial no Paraná entre 2012 e 2020. Trata-se de estudo descritivo e ecológico, com dados dos sistemas nacionais de informação do Brasil. Foram considerados os casos de gestantes e de crianças registrados entre 2012 e 2020 no estado do Paraná. Foram apresentadas as frequências absolutas e relativas para a caracterização, calculando-se a variação percentual entre o primeiro e o último triênio. Ainda, foi empregado o índice de Moran para a geoespacialização segundo regiões de saúde. Houve predomínio em mulheres de 20 a 39 anos (71,24%), brancas (67,22%) e com até oito anos de estudo (80,76%), com diagnóstico nas fases primária e latente (76,42%). Os casos concentraram-se em crianças do sexo masculino (48,72%), diagnosticadas na fase recente (96,42%), e naquelas cujas mães aderiram ao pré-natal (88,88%), mas os parceiros, por sua vez, não foram tratados (69,46%). Houve aumento do diagnóstico materno durante o pré-natal (16,61%) e redução dos óbitos infantis por sífilis (31,25%). Observou-se concentração das notificações nas regiões Metropolitana e de Pato Branco. Em suma, as gestantes apresentaram idade reprodutiva e baixa escolaridade e foram tratadas durante o pré-natal, contudo, sem a inclusão dos parceiros. Ademais, evidenciou-se comportamento espacial aleatório nas regiões de saúde, com disparidade entre a sífilis gestacional e a congênita.


Syphilis remains as serious public health issue due to existing obstacles in combating gestational and congenital syphilis. Since the onset of these infections is related to maternal and programmatic factors, this study analyzed the epidemiological profile of the mother-child binomial exposed to syphilis and its spatial distribution in Paraná from 2012 to 2020. This is a descriptive, ecological study with data from Brazilian national information systems. Cases of pregnant women and children recorded between 2012 and 2020 in the state of Paraná were considered. Absolute and relative frequencies were estimated for characterization, calculating the percentage variation between the first and last three years. The Moran index was also used for geospacialization according to health regions. Most women were aged 20 to 39 years (71.24%), white (67.22%), and had up to 8 years of schooling (80.76%), with diagnosis in the primary and latent stages (76.42%). Most cases concerned male children (48.72%), diagnosed in the recent phase (96.42%), and from mothers who adhered to prenatal care (88.88%), but the partners were untreated (69.46%). Maternal diagnosis increased during prenatal care (16.61%) and infant deaths by syphilis decreased (31.25%). Most cases were notified in the Metropolitan and Pato Branco regions. In short, the pregnant women were of reproductive age, had low education, and were treated during prenatal care, but their partners were not included. The health regions showed random spatial behavior, with disparity between gestational and congenital syphilis.


La sífilis persiste como un problema de salud pública, principalmente por los obstáculos en hacer frente la sífilis gestacional y congénita. Teniendo en cuenta que la ocurrencia de estas infecciones está relacionada con los factores maternos y programáticos, este estudio tuvo como objetivo analizar las características epidemiológicas del binomio madre-hijo expuestos a la sífilis y la distribución espacial en Paraná (Brasil) entre 2012 y 2020. Se trata de un estudio descriptivo y ecológico, con datos de los sistemas de información nacionales de Brasil. Se consideraron los casos de mujeres embarazadas y niños registrados entre 2012 y 2020 en Paraná. Se presentaron las frecuencias absolutas y relativas, calculando la variación porcentual entre el primer y el último trienio. Asimismo, se utilizó el índice de Moran, según regiones de salud. Predominaron las mujeres de entre 20 y 39 años de edad (71,24%), blancas (67,22%), con hasta ocho años de estudio (80,76%), con diagnóstico en estadio primario y latente (76,42%). Los casos se concentraron en hijos varones (48,72%), diagnosticados en fase reciente (96,42%), y en aquellos cuyas madres tuvieron acceso a los cuidados prenatales (88,88%), pero su pareja no recibió el tratamiento (69,46%). Hubo un aumento en el diagnóstico materno durante la atención prenatal (16,61%) y una reducción en las muertes de niños por sífilis (31,25%). Se observó una concentración de casos en las regiones Metropolitana y de Pato Branco. En resumen, las mujeres embarazadas se encontraban en edad reproductiva, tenían bajo nivel de estudios y eran atendidas durante el prenatal, sin incluir su pareja. Además, se evidenció un comportamiento espacial aleatorio en las regiones de salud, con disparidad entre sífilis gestacional y congénita.

ABSTRACT

Three-dimensional cone-beam computed tomography (CBCT) has an important role in the detection of vertical root fractures (VRFs). The effect of artifact generation by high-density objects like dental implants on image quality was well documented. This study aimed to assess the effect of tooth-implant distance and the application of metal artifact reduction (MAR) algorithm on the detection of VRFs on CBCT scans. This study was conducted on 20 endodontically treated single-rooted teeth. VRFs were induced in 10 teeth, while the other 10 remained intact. The implant was inserted in the right second premolar socket area, and two teeth were inserted in right canine and right first premolar sockets area randomly and underwent CBCT with and without the application of MAR algorithm. SPSS 21 was used to analyze the results (alpha=0.05). According to the findings of this study, all four variables of sensitivity, specificity, accuracy, and positive predictive values in diagnosis were higher in cases without MAR software at both close(roots in first premolar sockets) and far distances (roots in canine sockets) from the implant. However, the highest rate of diagnosis accuracy of the first and second radiologists was in the far distance group from the implant without MAR, and the lowest rate of diagnosis accuracy in the first and second radiologists was in the close distance to the implant. Applying MAR algorithm had no positive effect on detection of VRFs on CBCT scans in both close and distant scenarios.


La tomografía computarizada de haz cónico tridimensional (CBCT) tiene un papel importante en la detección de fracturas radiculares verticales (VRF). El efecto de la generación de artefactos por objetos de alta densidad como los implantes dentales en la calidad de la imagen está bien documentado. Este estudio tuvo como objetivo evaluar el efecto de la distancia entre el diente y el implante y la aplicación del algoritmo de reducción de artefactos metálicos (MAR) en la detección de VRF en escaneos CBCT. Este estudio se realizó en 20 dientes uniradiculares tratados endodónticamente. Se indujeron VRF en 10 dientes, mientras que los otros 10 permanecieron intactos. El implante se insertó en el área del alveolo del segundo premolar derecho, y dos dientes se insertaron en el canino derecho y en el área del alvéolo del primer premolar derecho al azar y se sometieron a CBCT con y sin la aplicación del algoritmo MAR. Se utilizó SPSS 21 para analizar los resultados (alfa=0,05). De acuerdo con los hallazgos de este estudio, las cuatro variables de sensibilidad, especificidad, precisión y valores predictivos positivos en el diagnóstico fueron más altas en los casos sin el software MAR tanto en distancias cercanas (raíces en las cavidades de los primeros premolares) como lejanas (raíces en las cavidades de los caninos) del implante. Sin embargo, la tasa más alta de precisión diagnóstica del primer y segundo radiólogo fue en el grupo de mayor distancia al implante sin MAR, y la tasa más baja de precisión diagnóstica en el primer y segundo radiólogo fue en la distancia cercana al implante. La aplicación del algoritmo MAR no tuvo un efecto positivo en la detección de VRF en escaneos CBCT en escenarios cercanos y distantes.

Purpose: The purpose of this study was to evaluate the onset of dissociated vertical deviation (DVD) and inferior oblique overaction (IOOA), their subsequent development, and their correlation with pre and postoperative parameters. Methods: Medical records of patients with infantile esotropia who underwent surgery between 2005 and 2017 were retrospectively reviewed. DVD and IOOA were measured before and after surgery. Patients were divided into two groups based on horizontal and vertical deviation at the time of presentation: those with infantile esotropia only (group A) and patients with infantile esotropia who developed vertical deviation (group B). Results: Out of a total of 102 patients, DVD occurrence was seen in 53 patients (51.9%) and IOOA was seen in 50 patients (48.04%). DVD was seen in 22 patients at the time of initial examination and in 31 patients postoperatively. IOOA at presentation was seen in 45 patients (44.1%) and 5 patients (8.8%) postoperatively. No statistical difference was found in the age of surgery, angle of deviation, mean follow?up, and mean refractive error within both groups. The postoperative motor outcome was statistically comparable between the two groups (P = 0.29). Sensory outcomes of fusion (P = 0.048) and stereopsis (P?value = 0.00063) were better in group A. Conclusion: No correlation was found between the age of occurrence and development of vertical deviation with refractive error, angle of deviation, age, or type of surgery. We found that motor outcomes are not affected but sensory outcomes are affected in patients with vertical deviations. This indicates that DVD and IOOA are developed due to inherent disruption of fusion and stereopsis

La mayor incidencia de la infección por el virus de inmunodeficiencia humana (VIH) en mujeres ha tenido un impacto directo en la transmisión vertical, situación que puede ser evitada con un adecuado control prenatal. Objetivo: Determinar características demográficas, epidemiológicas y obstétricas de madres de pacientes con exposición perinatal al VIH. Métodos: Se realizó un estudio retrospectivo, observacional, transversal y analítico. Se incluyeron madres con infección por VIH de transmisión horizontal, cuyos hijos con exposición perinatal, nacidos entre 2001 y 2020, fueron atendidos en la Unidad VIH del Hospital de Niños "J.M. de los Ríos" (Caracas-Venezuela). La información fue obtenida de la Base de Datos Interna. Las madres fueron agrupadas según la década de nacimiento del hijo (2001-2010 o 2011-2020). El análisis estadístico incluyó la prueba de Chi cuadrado. Resultados: Se estudiaron 805 madres. La edad promedio al nacer fue 26,4 años; el 8,6 % (n=69/803) era adolescente. El control prenatal fue inadecuado o inexistente en 59,7 % (n=463/776). La identificación de la infección materna fue obtenida durante o después del nacimiento en 36,4 % (n=280/769), con diferencias entre décadas: 26,7 % en la primera y 42,5 % en la segunda (p<0,01). En el 90,4 % (n=253/280) de este grupo el diagnóstico se obtuvo posterior al nacimiento. Conclusiones: La edad promedio de las madres fue 26,4 años. Aproximadamente 50 % tuvo control prenatal inadecuado o inexistente. Alrededor de un tercio obtuvo el diagnóstico después del embarazo, con significativo mayor porcentaje en la segunda década. Sólo en 1/10 madres de este grupo, la infección fue identificada al nacimiento.


The higher incidence of human immunodeficiency virus (HIV) infection in women has had a direct impact on vertical transmission, situation that can be avoided with an adequate prenatal control. Objective: To determine demographic, epidemiological, and obstetric characteristics of mothers of children with perinatal exposure to HIV. Methods: A retrospective, observational, cross-sectional and analytical study was carried out. It was included mothers, with horizontally transmitted HIV infection, whose children with perinatal exposure, born between 2001 and 2020, were treated at the HIV Unit of the Children's Hospital "J.M. de los Ríos" (Caracas-Venezuela). The information was obtained from the Unit Internal Database. The mothers were grouped according to the decade of her child's birth (2001-2010 or 2011-2020). Chi square test was performed for statistical analysis. Results: A total of 805 mothers were studied. The average age at birth was 26.4 years; 8.6 % (n=69/803) were adolescents. Prenatal care was inadequate or non-existent in 59.7 % (n=463/776). Identification of maternal infection was obtained during or after birth in 36.4 % (n=280/769), with differences between decades: 26.7 % in the first and 42.5 % in the second (p<0.01). In 90.4 % (n=253/280) of this group, the diagnosis was obtained after birth. Conclusions: The average age of the mothers was 26.4 years. Approximately 50 % had inadequate or nonexistent prenatal care. About a third were diagnosed after pregnancy, with a significantly higher percentage in the second decade. ABSTRACT

Background: Proper understanding of concepts of physiology forms basis of good clinical practice. To make the lectures in physiology more interesting and understanding, one has to review the teaching methods at regular intervals. Aims and Objectives: This study is designed to get feedback regarding teaching methods in physiology from MBBS students of Government Medical College, Kozhikode, to make teaching more effective. Materials and Methods: Two hundred randomly selected students who successfully completed 1st-year MBBS are included in the study group. The standard questionnaire is shared to study group through Google forms and adequate time is given to record their responses. Descriptive statistics are used. Frequency is expressed in percentage. Results: Most of students prefer interactive teaching methodologies such as group discussion as their preferred mode of learning rather than conventional didactic lectures in whole batch. Students also like to have integration with other subjects and early clinical exposure to understand the concepts precisely. Conclusion: Teaching in small groups with proper integration and alignment with other subjects will make maximum results rather than the traditional teacher centered medical education. Periodic review of teaching methods is very much essential to make the subject more interesting.

Context: Preparation of the root canal system is recognized as being one of the most important stages in root canal treatment which removes organic debris and microorganisms from the root canal system by means of chemico- mechanical preparation and irrigation of the canals. The use of nickel-titanium instruments has drastically reduced the time and the difficulties that were encountered with traditional hand instruments made up of stainless steel. Utilizing properties of super-elasticity, shape memory and different tapers of these instruments reduces not only the possibility of canal transportation but also affects both the geometry and volume of root canals. This subjects the root dentin to stress and consequently dentinal defects which increases the risk of root fracture during or after root canal treatment. Clinicians now have the opportunity to choose from differently tapered instruments having unique characteristics in their geometry and metallurgy. These are progressively tapered instruments, fixed tapered instruments, and variable tapered instruments, which come with the benefit of conforming to the root canal anatomy as well as removing dentin as little as possible while cleaning and shaping. The aim of this study was to examine the influence of instrument taperAim: on the fracture resistance of endodontically treated roots under in vitro experimental conditions.Conclusion: Under the limitations of this study in in-vitro conditions there were no significant differences between the fracture loads between the different file systems used, however samples prepared with Hyflex EDM recorded the highest fracture resistance, followed by ProTaper NEXT, ProTaper Gold and NeoEndo Flex respectively.

Objectives: The goal of this research was to see how reliable and valid three selected parameters from different analysis were in detecting the vertical skeletal pattern. Methods: There were 94 cephalometric x?rays used in all. The vertical skeletal pattern was assessed using the mandibular plane angle by Steiner, Frankfort mandibular angle by Tweed, and facial axis angle by McNamara. According to the diagnostic results of the majority of the measures, the samples were classified as normo?divergent, hypodivergent, or hyperdivergent. Kappa statistics, positive predictive value, and sensitivity were employed to verify the validity and reliability of the analyses. Results: A statistically significant weak negative correlation was found between Frankfort mandibular angle and facial axis angle in female group (p value < 0.01). A good agreement was found between mandibular plane angle with final diagnosis (K = 0.726). The mandibular plane angle showed highest sensitivity and positive predictive value in hypodivergent group (0.939, 0.816) and normo?divergent group (0.795, 0.833), respectively. Conclusion: For determining the facial vertical growth pattern the most accurate indicators were identified to be mandibular plane angle (SN?GoGn) and Frankfort mandibular angle.

Abstract Objective To evaluate the seroprevalence of toxoplasmosis among puerperal women cared for at a tertiary university hospital and the level of understanding of these puerperal women about toxoplasmosis, vertical transmission, and its prophylaxis. Methods For this cross-sectional study, we evaluated 225 patients using presential interviews, prenatal documentation, and electronic medical records. Data were stored using Research Electronic Data Capture (REDCap) software. Prevalence rates were estimated by the presence of reactive IgG antibodies against Toxoplasma gondii. Data analysis was performed using the chi-square test and calculation of the odds ratio (OR). Seroreactivity to T. gondii and exposure variables (age, educational level, and parity) were analyzed using a confidence interval (95%CI) and a significance level of 5% (p < 0.05). Results The seropositivity rate for T. gondii was 40%. There was no association between seroprevalence and age. Primiparity was a protective factor against seropositivity and low education was a risk factor. Conclusion Knowledge of T. gondii infection and its transmission forms was significantly limited, presenting a risk for acute maternal toxoplasmosis and vertical transmission of this protozoan. Increasing the education level regarding the risk of toxoplasmosis during pregnancy could reduce the rates of infection and vertical transmission of this parasite.


Resumo Objetivo Avaliar a soroprevalência de toxoplasmose entre puérperas atendidas em um hospital universitário terciário e o nível de compreensão dessas puérperas sobre toxoplasmose, transmissão vertical e sua profilaxia. Métodos Para esse estudo transversal, foram avaliadas 225 pacientes utilizando entrevistas presenciais, documentação de pré-natal e prontuário eletrônico. Os dados foram armazenados usando o software Research Electronic Data Capture (REDCap). As taxas de prevalência foram estimadas pela presença de anticorpos IgG reativos contra o Toxoplasma gondii. A análise dos dados foi realizada por meio do teste do qui-quadrado e cálculo do odds ratio (OR). A sororreatividade ao T. gondii e as variáveis de exposição (idade, escolaridade e paridade) foram analisadas, utilizando-se intervalo de confiança (IC95%) e nível de significância de 5% (p < 0,05). Resultados A taxa de soropositividade para T. gondii foi de 40%. Não houve associação entre soroprevalência e idade. A primiparidade foi fator de proteção contra a soropositividade e a baixa escolaridade foi fator de risco. Conclusão O conhecimento da infecção por T. gondii e suas formas de transmissão foi significativamente limitado, apresentando risco para toxoplasmose materna aguda e transmissão vertical desse protozoário. Aumentar o nível de escolaridade quanto ao risco de toxoplasmose durante a gravidez pode reduzir as taxas de infecção e transmissão vertical desse parasita.
